Online Form

Input can be provided via this online Input Form/Survey which requires MSU NetID/NetPassword authentication. MSU NetID/NetPassword authentication is required to prevent duplicate submissions and to ensure input is from relevant MSU stakeholders. Authentication information is not associated with the input data. This form will be open for input through February 2019. Therefore, there is no need to rush to complete the Input Form/Survey. It is recommended that you look over the form, participate in a town hall meeting, and reflect on the information. Then, when you are ready to provide input, log-in and submit your thoughts when you have the time to fully enter your responses. You will note that this form has seven (7) text fields (1,500 characters max) and five (5) survey items. You do not have to complete all fields, but it is requested that everyone complete the five survey items. Your responses are not recorded prior to you clicking the "Submit" button at the end of the form. Also, your answers will not be saved if you abandon the form prior to clicking submit.

Additionally, input can be sent to: Strategic-Plan@msstate.edu
